Insurance We use a special receipt which is accepted by most insurance companies when attached to your completed, signed insurance form. Therefore, you needn't bring forms to us for completion. We want to help you recover any money due you by your insurance company, but we generally will not bill insurance companies directly. It is your responsibility to provide payment and to recoup any reimbursement from your insurance company. If you are a member of a managed care program with which we participate, it is generally required that claims be submitted by us after collecting a co-payment from you. In order for your claims to be paid, an insurance authorization and your identification number must be provided at or before your first visit. Until that information is provided, we must follow our regular office policy of collecting the full charge at each visit.
